1 Acceptance of Terms
These Terms and Conditions (“Terms”) constitute a legally binding agreement between you (“Client,” “you,” or “your”) and Zen IV Therapy (“Company,” “we,” “us,” or “our”), a wellness clinic located at 5930 LBJ Freeway, Suite 370, Dallas, TX 75240.
By visiting our website at zenivtherapy.com, booking a consultation or service, or communicating with us via phone, SMS, email, or in person, you acknowledge that you have read, understood, and agree to be bound by these Terms and our Privacy Policy, which is incorporated herein by reference.
02 Description of Services
Zen IV Therapy provides intravenous (IV) nutrient therapy, vitamin infusion therapy, and related wellness services (“Services”) administered by licensed medical professionals in our Dallas, Texas clinic.
Our Services include but are not limited to:
- IV vitamin and mineral infusion therapy
- Customized nutrient drips for energy, immune support, hydration, and recovery
- Free consultations to assess individual wellness needs
- Post-viral recovery and fatigue support protocols
All services are performed by licensed nurses and trained medical professionals. Service offerings are subject to change without notice. We reserve the right to decline service to any individual at our discretion for health or safety reasons.
03 Medical Disclaimer
Important: The Services provided by Zen IV Therapy are wellness services and are not intended to diagnose, treat, cure, or prevent any disease or medical condition. IV therapy is not a substitute for professional medical advice, diagnosis, or treatment from a licensed physician or other qualified healthcare provider.
Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay seeking it because of something you have received at Zen IV Therapy.
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Zen IV Therapy is not an emergency medical facility.
Results from IV therapy vary by individual. We make no guarantees regarding specific outcomes. Testimonials and patient statements reflected on our website represent individual experiences and are not a guarantee of results.
04 Eligibility & Health Requirements
To receive our Services, you must:
- Be 18 years of age or older (minors require written parental or guardian consent)
- Complete a health intake form accurately and honestly before your first session
- Disclose all current medications, supplements, allergies, and pre-existing health conditions
- Not be pregnant or breastfeeding unless cleared by your physician
- Not have contraindications to IV therapy, as determined by our clinical staff
We reserve the right to decline or discontinue services to any client if, in the clinical judgment of our licensed staff, receiving IV therapy poses an unacceptable health risk. Our nurses may conduct a brief health screening prior to each session.
You are responsible for providing accurate and complete health information. Zen IV Therapy shall not be liable for adverse events resulting from incomplete or inaccurate health disclosures.
05 Appointments, Cancellations & Refunds
Booking Appointments
Appointments may be scheduled online at zenivtherapy.com or by calling (214) 351-2299. Your appointment is confirmed upon receipt of a confirmation message from us. We recommend booking in advance, as availability is limited.
Cancellation Policy
We ask that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment. Late cancellations (less than 24 hours’ notice) or no-shows may result in a cancellation fee. We reserve the right to implement and enforce cancellation fees at our discretion.
We reserve the right to cancel or reschedule appointments due to staff availability, equipment issues, or circumstances beyond our control. In such cases, we will contact you as soon as possible to reschedule.
Refund Policy
Prepaid services are generally non-refundable once a session has been administered. If you are dissatisfied with your experience, please contact us at (214) 351-2299. We will review your concern and work to find a reasonable resolution on a case-by-case basis. Package purchases that are unused may be subject to a partial refund at our discretion.
06 Payment Terms
Payment is due at the time of service unless prior arrangements have been made. We accept major credit cards, debit cards, and other payment methods as displayed at the time of booking or service.
IV therapy services are generally not covered by health insurance. You are solely responsible for all fees associated with our services. We do not submit claims to insurance providers on your behalf.
All prices are subject to change without notice. Pricing in effect at the time of your appointment will apply. Any applicable taxes will be added to your total.
07 SMS Communications
About Our SMS Program
Zen IV Therapy — Zen IV Therapy is a licensed IV nutrient therapy and wellness clinic located in Dallas, TX. When you opt in to our SMS program, you can expect to receive text messages related to: appointment reminders and confirmations, follow-up care check-ins, wellness tips and service updates, and promotional offers about our IV therapy services (only if you have specifically opted in to marketing messages).
SMS Data Policy: No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
How to Stop Receiving Messages
You can cancel the SMS service at any time. Just text “STOP” to (214) 351-2299. After you send the SMS message “STOP” to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.

Carrier Liability
Carriers are not liable for delayed or undelivered messages.
Message & Data Rates
As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive message frequency varies per user based on interactions and appointment activity. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.

Permitted Use

- Any adverse health reactions that result from incomplete or inaccurate health disclosures made by the client
- Service interruptions, errors, or delays beyond our reasonable control
- Unauthorized access to your personal information despite our reasonable security measures
In all cases, our total liability to you for any claim arising from these Terms or our services shall not exceed the total amount paid by you for the specific service giving rise to the claim.
Some jurisdictions do not allow the exclusion or limitation of certain damages, so the above limitations may not apply to you.

If negotiation is unsuccessful, disputes shall be resolved through binding arbitration in Dallas County, Texas, in accordance with the rules of the American Arbitration Association, unless you and we both agree to proceed differently.
You waive any right to participate in a class action lawsuit or class-wide arbitration against Zen IV Therapy.
Notwithstanding the above, we reserve the right to seek injunctive relief in any court of competent jurisdiction to protect our intellectual property rights.
